Register online with the Pennsylvania Department of Health
You will need to register here with your driver's license. You will receive a patient ID number that the doctor will need prior to issuing certification for medical marijuana treatment. Once you have your patient ID number, fill out this evaluation form. We will contact you the same day to schedule an appointment to speak with the doctor. The professional fee for the medical evaluation is $199. The payment must be made prior to your appointment.

Complete registration and receive your card
After certification, you will need to login to the DOH account used to register and click "make payment" to submit a $50 fee to the PA Department of Health. The DOH will mail you a medical marijuana card in a week. Visit any dispensary in Pennsylvania with your card to purchase medical marijuana products. Consultation with a dispensary pharmacist is highly recommended.
